Medical Mistakes in Hospitals

Your local hospital just might be more of a death trap than an actual health care facility. A new report issued by the Office of Inspector General at the Health and Human Services Department (HHS) says that every month roughly 134,000 hospital patients experience some type of adverse event during their stays. And about 15,000 of them die every month due to various medical and surgical errors.

According to the report, such adverse events include surgery mistakes, medication dosage errors, improper care protocols, and transmission of infection due to filthy conditions. In fact, patients are often admitted to hospitals in healthier condition than when they leave (if they even do), and much of the time their health declines are a result of avoidable medical errors.

Many hospitals are also hotbeds of infectious diseases like Methicillin-resistant Staphylococcus aureus (MRSA), the infamous hospital "superbug" that kills roughly 48,000 people every year (http://www.naturalnews.com/028441_s...).

Nearly 50 percent of all adverse event problems are avoidable, according to the report. But since there is no tracking system in place to address the problems and work towards fixing them, many hospitals continue to make them without consequence -- and the vast majority of hospital patients have no idea about the significant risks they face.

"This report shows that hospital patients are being harmed by medical errors at an alarming rate," explained Lisa McGiffert from Consumers Union, in a statement. "Unfortunately, most Americans have no way of knowing whether their hospital is doing a good job preventing medical errors."

At this time, there is no solid way to determine where hospitals rank in terms of overall safety performance because no reporting system exists for tracking this information. But the report urges both the Agency for Healthcare Research and Quality and the Center for Medicare and Medicaid Services to work towards implementing one.

Firefighters and medics may be, perhaps not surprisingly, at a higher risk for carrying methicillin-resistant Staphylococcus aureus (MRSA) than the average person, according to results from a new study conducted by Marilyn Roberts, a University of Washington professor of environmental and occupational health sciences. Roberts, a microbiologist, recently conducted the first-ever environmental health study on MRSA in Northwest fire stations and on fire personnel to determine the extent of related contamination.
In the last ten years, the number of hospital- and community-acquired MRSA infections--those often contracted in schools, public gyms, and in workplaces--has risen. Because MRSA can be transmitted from surfaces to people and from person to person, the increase in incidence has led to concern for first responders, including police, firefighters and emergency medical personnel. Fire personnel interact with both hospital and community populations as part of their job and have the potential to be exposed to MRSA as part of their daily duties.

"Firefighters and paramedics are at the crossroads between the public and hospital environments," said Roberts. "Their job includes administering first-response care to patients, many of whom are more likely to be MRSA carriers or have MRSA infections then the general population. This puts them at increased risk for MRSA infections."

Roberts found MRSA in four percent of more than 1,000 samples collected from surfaces inside Snohomish County fire stations, medic and fire trucks, and outer protective gear. But in each of the nine areas sampled, at least one sample tested positive for MRSA, and both hospital- and community-acquired MRSA were found.

Roberts conducted the study after being contacted by firefighters who asked for help in determining whether they were being exposed to MRSA on the job. "Firefighters and medics were part of an advisory board that helped us develop the study," she said.

Firefighter and paramedic Kevin Fetter said that he had questions about a new disinfection system, including: What was on the surfaces or being transmitted that might cause infection or illness? As the wellness coordinator for Snohomish County Fire District 1, Fetter called labs all over the country asking for help in testing before connecting with Roberts at the University of Washington. The district is made up of 12 fire stations that field 13,000 emergency calls each year.

Fetter and colleagues knew that MRSA was being studied in hospital settings, but didn't know how MRSA affected them. Except for a small study in Arizona, no one else had looked at this setting and firefighter exposure.

"We now recognize it as a problem," said Fetter, referring to the firefighter community. The UW study results have attracted attention in other regional fire departments, since findings showed firefighters can be carriers.

The research team found more than 20 percent of the fire personnel who volunteered to be tested carried MRSA and an additional 10 percent carried Staphylococcus aureus in their noses. Normally, 25 to 35 percent of the general population carries S. aureus in the nose. Also, in studying the genetic characteristics of the different strains of bacteria, Roberts found that MRSA strains on fire personnel were related to the environmental MRSA strains, suggesting transmission between personnel and the environmental surfaces.

Roberts and her team looked at MRSA contamination in fire stations and determined which locations were most likely to be contaminated. The study also was conducted to improve decontamination of the station and truck environmental surfaces and to use the information to develop and distribute educational materials on best practices for disinfection protocols.

Roberts said she hopes to expand the study to determine if the results from the initial study are representative of the larger firefighter population within Washington state and throughout the United States.

Funding for the project was sponsored by the Washington State Department of Labor and Industries' Safety & Health Investment Project (SHIP) and from Snohomish County Fire District 1. SHIP funding is directed at preventing workplace injuries, illnesses, and fatalities in Washington state.

DIrty money: You may be collecting more than just change when you're handed back dollar bills. Germs also live on money, potentially spreading bacteria that can cause strep throat and staph infections.

Paper money usually comes in contact with several different people every day, allowing it to collect germs and bacteria. A 2001 study by Dr. Peter Ender found that 86 percent of the bills he studied had germs such as Streptococcus, which causes strep throat, while 7 percent of bills had more harmful bacteria, such as Staphylococcus, which causes staph infections. Only 7 percent of the bills he studied were germ-free.

Jeff Dimond, a spokesman for the Centers for Disease Control and Prevention, says it is possible for money to spread germs since bacteria can live on surfaces for several hours. If someone uses the bathroom without washing their hands or sneezes and then gives you money, all of those germs could be passed on to you.

To help prevent the spread of germs and diseases, Dimond suggests getting vaccinated, washing you hands often, using hand sanitizers and keeping your hands away from your nose and mouth.

Silver

It has been known for centuries that silver has disinfecting properties. Silver was used as a germicide as long ago as Greek and Roman times. CHICAGO — The number of children hospitalized with dangerous drug-resistant staph infections surged 10-fold in recent years, a study found.
Disease incidence increased from 2 cases to 21 cases per 1,000 hospital admissions from 1999 to 2008. Most infections were caught in the community, not in the hospital.
The study involved methicillin-resistant staph infections, called MRSA. These used to occur mostly in hospitals and nursing homes but they are increasingly showing up in other settings in children and adults. Recent evidence suggests hospital-acquired MRSA cases may be declining while community-acquired cases are becoming more common.
The results are "a good example of how something that is not unexpected remains alarming," said Dr. Buddy Creech, an infectious disease specialist at Vanderbilt University. He was not involved in the study.
The study involved 25 children's hospitals; the 10-fold increase in hospitalizations likely occurred nationwide, said Dr. Jason Newland, the lead author and an infectious disease physician at Children's Mercy Hospitals and Clinics and the University of Missouri-Kansas City.
Almost 30,000 children were hospitalized with MRSA infections at the hospitals studied during the 10-year period. Most had skin or muscle infections, and 374 youngsters with MRSA died. While Newland said it isn't clear if MRSA caused those deaths, it can be deadly and is blamed for more than 18,000 deaths in children and adults nationwide each year.
The study didn't examine whether deaths or the severity of infections increased.
The results were published Monday in the journal Pediatrics.
MRSA often begins as a pimple or boil on the skin. It can also spread to other parts of the body, including the bones or lungs, where it can cause pneumonia.
The study also found a coinciding increase in use of clindamycin, an antibiotic that comes in easy-to-use pills and liquid, and smaller increases for two other antibiotics. Another drug effective against MRSA, vancomycin, is only available intravenously and its use decreased during the study.
Newland said the increasing use of clindamycin is concerning because in some regions MRSA is already becoming resistant to the drug. Doctors need to use the antibiotic judiciously, he said.
Dr. Kenneth Alexander, the University of Chicago's pediatric infectious disease chief, said he agrees.
"Staph are incredibly cagey, and will ultimately find their way around any antibiotic in use," he said.
Research is needed to find other drugs that will work against MRSA, he said.
